Students are responsible for providing current documentation of their disability and the need for accommodations. This documentation must identify the disability for which the accommodations are requested, and substantiate that the need for accommodations is related to the disability.
Federal and state laws protect individuals with disabilities from the release of personal information without their consent. In order for Disability Support Services to either receive documentation of a disability or to share information regarding a disability, a confidentiality-release form must be signed by the student. This release statement is only good for one year, and is kept in the student's locked file. Student records, including documentation, will be kept for 5 years from when the student becomes inactive.
Nicolet College and Disability Support Services coordinate testing for learning disabilities and attention deficit disorder with a professional psychologist. Students who need to update their testing or wish to be tested for LD or ADD may be eligible to have the testing done through Nicolet at no cost to the student. Students must see a case manager to determine if they fit eligibility requirements for the
testing.
If a student/prospective student disagrees with a decision regarding some aspect of their disabilities request, they may appeal the decision to the Vice President of Human Resources or designee within 30 calendar days using the following procedure:
